KishHealth System’s Advance Care Planning team, now part of Northwestern Medicine, awarded Kathy Heffley with a Fitbit. Heffley was entered into the drawing after participating in the annual This One’s for the Girls event at Kishwaukee Hospital. During the event she signed up for a follow-up appointment with an ACP facilitator to discuss her Advance Care Plan. Participants who attended appointments were entered into the drawing for the Fitbit. Heffley was chosen as the winner.
Advance Care Planning is a process of coming to understand, reflect on, discuss, and plan for a time when you cannot make your own medical decisions and are unlikely to recover from an injury or illness. Effective planning is the best way to make sure an individual’s views are respected by loved ones and healthcare providers. This process also provides comfort to those who may make end-of-life decisions for their loved ones. Good advance care planning improves the quality of advance directives.
