KishHealth System encourages patients to sign up for MyKishHealth, the online patient portal for Kishwaukee and Valley West Hospitals, to access to their medical information. Designed to give patients direct online access to portions of their patient medical record, patients can review laboratory and radiology results, billing statements, visit history, and request appointments. All information is securely stored in this electronic record for quick, easy and secure access by the patient or a designated proxy.
Patients can sign up for MyKishHealth at the time they register for a service or procedure at Kishwaukee Hospital or Valley West Hospital. Patients will need to confirm some basic personal data, and then will receive a card with their medical record number and instructions on how to set up their new accounts. Patients can also bring a photo ID to the Medical Records department at either hospital, provide a valid email address to be sent a link to set up their patient portal account. Additionally, a form is available online and can be submitted electronically, through fax or mail delivery. For more information about MyKishHealth, online patient portal, visit www.mykishhealth.org.
